Miscellaneous Fees

Passports

  1. Form must be filled out in black ink - No whiteout.
  2. 2X2 pictures (available at Rite-Aid, Meijer, Walgreens, etc).
    • Or Passport Photographs for $10.00 at the Clerk of Court's Office
  3. Certified copy of Birth Certificate.
  4. Valid Driver's License or State ID.
  5. If under the age of 16 then both parents must be present with child.
  6. For child's passport, if parent's name on driver's license does not match birth certificate must show documentation of change (marriage license).

Passport Photographs for $10.00 at the Clerk of Court's Office

Adult - Passport

Card

  • $30 to US Department of State (check or money order only) - NO CASH

Child - Passport

Passport book by Mail DS-82 is $130.00 with/no execution fee charged when adults 16 and over with fully valid passport books apply for the passport card.

* It is taking approximately 4 to 6 weeks for the passport to come back, be sure to allow yourself enough time. It can be expedited for a $78.32 fee, plus cost of overnight envelopes to send it.
