Passports

  1. Form must be filled out in black ink.
  2. 2x2 duplicate pictures.
  3. Certified copy of Birth Certificate
  4. Valid Driver's License or State ID
  5. If under the age of 14, then both parents must be present with child.

Passport book by Mail DS-82 is $110.00 with/no execution fee charged when adults 16 and over with fully valid passport books apply for the passport card

* It is taking approximately 4 weeks for the passport to come back, be sure to allow yourself enough time. It can be expedited for a $60.00 fee, plus cost of overnight envelopes to send it.
